Lockout/Tagout Tags: The 1910.147 Rules That Get Cited

The Control of Hazardous Energy standard sits in OSHA's most-cited top ten every year, and the tag itself is where surprisingly many programs fail — not the locks, not the procedures, the tag: hand-scrawled cardstock, string attachments, faded legends, three different formats in one plant. The tag requirements are five short rules. Here they are, with the material engineering that satisfies them.

1. The Five Tag Rules

All from 29 CFR 1910.147(c)(5) — short enough to audit against directly:

2. Printed Tags Beat the Marker Pen

The standardization and legibility rules are an argument for printing: a tag generated with the authorized employee's name, the equipment ID, and the date — in the facility's one standard format — is auditable at a glance and survives what handwriting does not. The material stack that works:

  • Tag body: heavy polyester tag stock with a reinforced eyelet — resists oil, coolant mist, and washdown chemistry that turns cardstock to pulp. The ANSI Z535.5 DANGER header format is the recognized standard styling.
  • Print: thermal transfer with resin ribbon, so the legend survives solvent contact — the same chemistry argument as the GHS container guide.
  • Attachment: one-piece nylon cable tie meeting the 50-lb benchmark, threaded through the eyelet — never string, never wire.
  • Printer: any 4-inch benchtop handles tag stock; an i5300-class machine prints the red DANGER header and the black variable data in one pass.
The tag is the warning; the lock is the protection

1910.147 is explicit that tags are warning devices without the physical restraint of a lock. Beyond the tag itself, the catalog's Prinzing-line lockout devices — ball valve lockouts (polypropylene, -20°C to +80°C), pneumatic quick-disconnect lockouts (glass-filled nylon, to +120°C) — provide the actual energy isolation the tag announces.

3. Tag Program Audit Checklist

  • One tag format facility-wide — pull three tags from three departments and lay them side by side.
  • Every tag identifies its applier legibly, with a date.
  • Attachment means on the tag cart are one-piece nylon ties, and nothing else is available to grab.
  • Tags on wet or corrosive equipment are polyester, not laminated paper.
  • Energy isolation points are themselves labeled (equipment ID + energy source) so the procedure and the hardware match — printed equipment ID labels close that loop.
  • Annual periodic inspection includes tag condition, not just procedure steps.
